Call during business hours to make an appointment. We accept same-day patient appointments for acute care. If you need to cancel or reschedule an appointment, please call 24 hours in advance. A cancellation within an hour of your appointment is considered a no-show and carries a $25 charge.

What to Bring -- Bring your insurance cards to each appointment and any forms for the physician to complete. We will verify your personal information and make a copy of your current insurance information. We can provide work/school notes documenting your visit.

New Patients -- Please arrive 15 minutes prior to your appointment time to complete registration forms and a health history. Our health history forms

are available to print, complete and bring with you to the appointment. Please bring a picture identification card or driver's license to be copied and placed in your chart.

If you are considering having your medical records transferred to Florissant Valley Family Medicine, please discuss this with your physician before authorizing the transfer. Many offices charge to transfer records, and it may be unnecessary to transfer your entire record.

Telephone Calls -- Call during business hours to schedule appointments, ask questions or request a referral. If you are experiencing a medical emergency, call 911 or go to the nearest Emergency Department. For urgent care needs that cannot wait until our regularly scheduled hours, call Nurse Call Consultants at 314.972.9292.

Cancellations and Late Arrivals -- We appreciate your prompt notice if you are unable to keep your appointment or if you anticipate being late. That way, our staff can offer a canceled appointment time to another patient. If you are running late, the staff can determine if the physician's schedule allows him or her to see you that day or if you will need to re-schedule.

Prescription Refills -- It is not necessary to call our office for most prescription refills. Call your pharmacy and give them the prescription number from the bottle. The pharmacy will fax our office for the refill request. Call us if your insurance plan requires a written prescription. Allow 48-72 hours to fill prescriptions. Nurse Call Consultants cannot authorize prescription refills.  

Referrals -- Please allow 48 hours to process requests for referrals to specialists with your insurance carrier. We are unable to refer you for conditions that have not been evaluated by our physicians.
